Many of the WIAA forms that have typically been sent to schools are now available online to download.   All forms are available in three formats: Adobe PDF, Microsoft Word for Windows, and Microsoft Word for Macintosh. 

Adobe PDF - A file format that only requires the Adobe Acrobat Reader software to be installed in order to view.  Acrobat Reader is free from Adobe, and can be obtained by clicking the graphic below.  Once the file is downloaded from Adobe, you will need to open the file "ar40" (or similar) to install the reader on your hard drive.  Be sure to select that you are using the program to view web pages, when asked.


Microsoft Word for Windows - The files were created in Word 97 and are viewable in both Word 97 and Word 2000.

Microsoft Word for Macintosh - These files were converted from Word 97 to Word 98 for Mac, Word 6.0 for Mac, and Word 5.x for Mac using conversion software. 

How to download files:

  1. Find the form you want from the list below.  Next to each form is a set of five icons - and the icons are:
    - Adobe PDF format   
    - Microsoft Word format  
    - Microsoft Excel format
  2. Click on the icon that represents the file format you wish to download.  You will be transferred to the WIAA FTP (file transfer protocol site), and the file will download to your machine.
  3. Click "Print" under the File menu if you wish to print the document, or "Save As..." under the file menu to save the document to your machine.
  4. That's all there is to it!

Combined - Coop Form:                                    
Application for two or more to enter into a combined or cooperative program.  Must be filed with the WIAA by the end of the third week of the season.

Game Contract:                                  
Contractual form for two or more schools to agree to a contest.  A great way to not overlook a booking or double-book!

School Board/Superintendent Pass Application:               
Form for identifying the Superintendent and School Board members as supervisors as state events.

WIAA/WSFA Participating School Forensic Tournament Joint Supervision Agreement 
Agreement form for identifying the supervisor for forensics tournament participants when two or more schools to be supervised by a coach from a single school.

Wrestling Weight Permit Form:   High School:                MS/JH:                            
The form for individual wrestlers to have filled out by a physician establishing minimum wrestling weight.

Wrestling Weight List for Matches:   High School:                 MS/JH:              
Weigh-in form for dual matches with actual weight and Dr. minimum recommendations.

Wrestling Weight Certification Summary Form:            
Maintain a list of doctor's minimum recommended weights to protect yourself and your athletes.

Wrestling Participation "W-2" Form          
Senior High summary of season participation identifying certified weight, number of weigh-in at each wrestling weight, and won-loss record.  Typically used in preparation for post-season tournaments.

Scholastic Award Worksheet:                                 
If a team or activity group has a combined GPA of 3.0 or higher, they are eligible for a WIAA team/activity scholastic award.  The highest combined GPA's for each sport in each classification qualifies as state Academic Champion! Note: These forms are worksheets only; beginning August 2003 only orders submitted online will be processed for awards.

WIAA Clock Hour Program Application:          (if asked to give a network password, click the 'Cancel' button - that will get you to the form)
If you wish to propose a program for WIAA/OSPI Clock Hour credits, this is the form that needs to be used.  IMPORTANT: Completed applications MUST be received at the WIAA office no less than 14 days prior to the program in order to be approved!

WIAA Amendment Proposal:                                  
The form for recommending changed to the WIAA Handbook.  Must be submitted by December 1.

Gareth Giles Nomination Form:                                 
Gareth Giles was a long-standing advocate of scholastic activities in the State of Washington, and this award is to recognize those who hold a similar passion by participation in leadership activities throughout the state.  Must be submitted by December 1.

WIAA Regional/State Site Application:                                 
For schools/facilities interested in hosting a WIAA tournament.

Just Play Fair! Manual, including Applications :
Just Play Fair! is the WIAA Sportsmanship program.  Schools or leagues may apply for the Just Play Fair! program to show their dedication to sportsmanship.  (Use the worksheet to help fill out the application).        

Just Play Fair! Certificate:
The Just Play Fair! Certificate is a great way to recognize someone who has shown an exemplary display of sportsmanship.

Medical Emergency Authorization Form:                                 
This sample form is an outline to assist schools in creating a medical release form.

WIAA Physical Form:                                 
The standard pre-participation physical history and authorization form.

WIAA Physical Update:                                  
Once the physical form is on file, only the physical update is needed yearly to maintain a student's physical eligibility to participate.

Wrestling Skin Condition Evaluation Report:
Physicians' evaluation report authorizing a wrestler to compete while displaying a non-communicable skin condition.

Youth Football Heat Risk Guidelines:
The published guidelines,  Youth Football: Heat Stress and Injury Risk, are based on recommendations from a national panel of experts who examined hydration, environment, and other factors which cause players to overheat.  The recommendations are designed for coaches, parents, and players to follow during preseason football practices.
